Job Description

Primarily responsible for managing early stage engagement programs, caregiver support group program, and respite care grants. The Family Program Manager will also manage community outreach, promotion of Association services, constituent inquiries, and partnership development in assigned Chapter territory.

Essential Job Functions

 
     
  • Early Stage & Caregiver support Programs 
  •  
  • Manages the program design and implementation of early stage engagement and caregiver support group programs, including delivery of education programs, support groups, and socialization activities
  •  
  • Manages a team of volunteer facilitators, including recruitment, screening, orientation and on-ongoing training, individual and group supervision, and recognition
  •  
  • Provides administrative direction and support to ensure effective operations of support groups and early stage community 
  •  
  • Develops strategic partnerships with community groups to grow program opportunities, participant outreach, volunteer recruitment, and provision of program space
  •  
  • Ensures collection for program data and performs periodic data analysis to inform future program growth
  •  
 

Respite Care Grants

 
     
  • Manages the assessment and application process for respite care grants
  •  
  • Evaluates applications, conducts applicant interviews, and makes recommendation for approval of grants
  •  
  • Manages communication among grant recipient, agency or facility contacts, and Chapter staff
  •  
  • Tracks respite care grant data in partnership with other staff 
  •  
 

Relationship & Partnership Development

 
     
  • Identifies and expands opportunities for partnership with community organizations and businesses to support strategic programmatic goals 
  •  
  • Facilitates Information & Referral sessions and Care Consultations with constituents 
  •  
  • In partnership with Development staff, serves as the primary representative and liaison for the Chapter in assigned region
  •  
  • Provides leadership and/or support as needed for special events, conferences, both internal and external to the organization
  •  
 

Minimum Requirements

 
     
  • Bachelor’s degree in health care or social services-related field; Master’s degree preferred 
  •  
  • Three to five years of experience in health care or social services, particularly specialties in aging or long-term care 
  •  
  • Ability to work with a wide range of individuals, including persons with dementia, family caregivers, physicians, private and public partners, volunteers, legislators, and public policy officials
  •  
  • Excellent organizational skills with strong attention to detail 
  •  
  • Office hours are 9:00 am to 5:00 pm Monday through Friday  
  •  
  • Occasional evening and weekend hours and travel throughout Chapter territory to fulfill work requirements
  •  
  • Workload will fluctuate with varying deadlines and priorities
  •  
  • Work may be conducted in a variety of environments, including office and public meeting spaces
  •  
  • Bilingual (English/Spanish) desired 
  •  
  • Must have a valid driver’s license and a willingness to use personal vehicle in the course of employment (must show proof of automobile insurance)
  •  
  • Strong written and oral communication skills 
  •  
  • Proficient in Microsoft Office products, especially Word, Excel, Power Point, and Google applications
  •  
  • Self-starter who has the ability to work independently and as part of a team
