General Summary: 

Working as a member of the clinical services department, the Clinical Services Coordinator provides excellent care consultation services to people affected by dementia.  The goal of this position is to provide quality care consultation service to families in our service area with appropriate and timely documentation.  This position represents the chapter at county coalition meetings as requested by Clinical Services Director. 

This is a part-time position (30 hours a week) located at our Miami Valley (Dayton, OH) Chapter.

Essential Job Functions:

  • Provide quality care consultation services to people affected by dementia, including appropriate documentation and database utilization
  • Engage the community in the mission and vision of the Association through attendance at events, meetings and other opportunities
  • Present education programs as needed related to first steps series and community education and professionally  represent  the  chapter through attendance and reporting at meetings as requested by Clinical Services Director
  • Provide helpline support as needed
  • Coordinate the strategic direction of the Early Stage program in conjunction with the Clinical Services Director
  • Candidate must be able to exercise excellent judgment, accept responsibility, and give careful attention to details. Excellent oral and written communications.
  • Develop long-range (1 year) and short term (3 months) objectives and plans to meet them
  • Must develop working knowledge about Alzheimer's disease and its impact on families and caregivers.
  • Proficiency in Microsoft Office specifically Word, Power Point and Excel and Google apps specifically Google Sites, Docs, Drive and Calendar
  • State of Ohio driver's license, proof of insurance and access to dependable automobile
  • Frequent travel within the nine-county service area (Montgomery, Greene, Clark, Champaign, Logan, Preble, Miami, Shelby, and Darke); Occasional overnight travel

Minimum Qualifications:

  • Masters Degree in social work or related field 
  • Minimum of 3 years relevant work experience 
  • Experience in case management work with families Excellent interpersonal skills, excellent oral and written communication skills
  • Ability to plan and work independently Public speaking experience
  • Flexibility to work evenings and weekends Superior organizational skills
  • Experience working with client type data base systems
  • Program coordination skills
  • Experience working with vulnerable populations/people with dementia
  • Prioritize own work
  • Delegate work to others
  • Check the quality and timeliness of own work
  • Autonomously work in the field, some evenings and weekends
