Care Services Manager
The Care Services Manager will actively participate in the Central New York program team to ensure effective execution and completion of strategic goals, objectives and program work plans. This position is responsible for the implementation and delivery of core services, as well as expansion of reach through volunteer-powered program delivery. This position is responsible for developing and deploying a plan to recruit, onboard, train, oversee and recognize program volunteers.
Essential functions and responsibilities include, but are not limited to:
Plan, execute, document and evaluate core services in the chapter’s coverage area, including education programs, support groups, and social engagement. Exercises discretion to execute programs and services department goals and objectives.
Oversee Alzheimer’s Association volunteers to assure expansion of quality programs. Primary responsibility for annual performance evaluations of volunteer positions for community educators, support group, social engagement. Performance evaluation will be in concert with functional team leads.
Engages, trains and manages relationships with volunteers to expand community visibility and service delivery, especially among racial/ethnic communities, high risk and underserved populations.
Provide care consultation services to individuals living with dementia, their family, and/or care partners. Develop customized needs assessments including establishment of person-centered care plans, provision of emotional support and education, and documentation of progress.
Represent the Chapter at community events, including but not limited to, community outreach events, volunteer association meetings, health and wellness fairs and volunteer fairs.
Analyze, collect and enter relevant and appropriate data for grant reporting, measuring progress toward strategic goals, and submitting monthly reports to the Senior Director of Programs.
Keep informed of current disease information, current initiatives of the chapter and national offices via online technology, journals and other sources provided.
Assure compliance with all Association policies, procedures, Standards and applicable regulatory and grant requirements; promote Association wide programs and initiatives
Bachelor’s degree required in human services, gerontology, social work or a related field; Master’s degree preferred.
Three years of experience working in health and human services related field.
One to three years of leadership experience in program administration including needs assessment, program development, delivery, management and evaluation.
Demonstrated success developing and leading a high-performing team of volunteers to achieve strategic initiatives.
Demonstrated success managing volunteer-led, consumer-facing programs with experience in recruitment, training, engagement, supervision and evaluation of volunteers.
Demonstrated success working with diverse populations, volunteers, staff, and external contacts to build and maintain successful teams.
Bilingual (English & Spanish) preferred.
Must have a clean NYS driver's license, access to a vehicle, and ability to travel throughout coverage area.
Knowledge, Skills and Abilities
Knowledge of best practices of managing volunteers.
Excellent verbal and written communication skills.
Ability to multi-task and prioritize work.
Ability to take feedback, work independently as well as work as a member of a team.
Goal-oriented, motivated to achieve all key performance indicators (KPI’s)
Familiarity with Alzheimer’s and related dementias, and/or aging and health issues strongly preferred.
Proficiency with Microsoft Office applications, especially Word, Excel, and PowerPoint.
Ability to carry up to 25lbs of equipment and supplies.
Must have a valid driver’s license, reliable transportation and ability to travel by car throughout the fourteen-county region of Central New York with occasional out-of-area travel.
