Position Summary:

The Alzheimer's Association, Long Island Chapter provides a variety of education, information and support services to persons with dementia, their families, health care professionals, and the community.  The Care Services Manager will implement care/support services, including Information and Referral and Care Consultation services to constituents residing within Nassau and Suffolk Counties.  The Care Services Manager is also responsible for the oversight of the Chapter's support groups (Caregiver, Early Stage, Younger Onset), and for the recruitment, training, retention and oversight of volunteers facilitating existing or to-be­ developed support groups (caregiver and for people living with dementia early stage).  The Care Services Manager will assist the chapter in its outreach activities to the physician community diagnosing and caring for people with Alzheimer’s disease and other dementias. This position may operate off-site several times each month to offer walk-in care consultation services, support groups and outreach to physician offices.

Responsibilities

Essential functions and responsibilities include, but are not limited to:

Advancing Care and Support

  • Conduct Information and Referral (I&R) sessions and/or Care Consultation (CC) sessions as indicated by constituent need, on and off-site.
  • Provide individual and family care planning services.
  • Assist clients in accessing services and resources.
  • Complete required training and practice for persona assignment- Personify 360 (CRM).  Document I&Rs/CCs in Personify as outlined in The Helpline Standards.
  • Conduct program evaluations.
  • Assist in the chapter’s physician outreach initiatives by conducting visits and building relationships with physicians, nurses, office staff, and clinics in Nassau and Suffolk Counties.  
  • May provide clinical supervision to junior staff and supervision to office volunteers.
  • Develop and update relevant printed materials.
  • Prepare and conduct community presentations to expand community awareness of Alzheimer's disease, other dementias and the Alzheimer's Associations' programs and services.
  • Maintain knowledge and referral base through networking with other professionals.
  • Represent the Alzheimer's Association at public forums, conferences, and media events; function as an advocate representing the Alzheimer's Association in the community.
  • Assist in the marketing of agency programs and services.
  • Assist supervisor as needed

Volunteer Oversight and Development

  • Recruit, train, retain and coach (staff partner) volunteer support group facilitators.
  • Provides initial screening and support for volunteer support group facilitators. 
  • Provides coordination of support groups.  

Operational Effectiveness

  • Works with staff to assure that requirements for grant-funded programs and services in the assigned territory are met.
  • Ensures new community resources and related events are submitted to helpline databases.
  • Maintains accurate records of program and event participation and other metrics.
  • Manages activities within budgeted parameters. 
  • Works with the national office to ensure uniformity of materials and presentation content.
  • Submits timely and accurate records and reports, newsletter articles and other information as requested. 

Qualifications  

  • Master's degree in Social Work or related field, or commensurate experience
  • MSW, LMSW preferred.
  • Bilingual preferred. 
  • Minimum of 2 years experience in counseling 


Knowledge, Skills and Abilities 

  • Working knowledge of community resources and Alzheimer' disease and other dementias preferred
  • Excellent interpersonal and verbal/written communication skills
  • Demonstrated success working in volunteer-led programs, with experience in recruitment, training, engagement, and supervision of volunteers
  • Strong organization skills and proficiency in maintaining systems and procedures 
  • Proficiency in Microsoft Office and Google Apps
  • Ability to analyze information and make timely, appropriate decisions
  • Appreciation of cultural and ethnic diversity, especially with respect to working with underserved communities
  • Experience with crisis and risk assessment preferred 
  • Experience with CRM system software preferred
  • Driver’s license and access to a car is required
