The mission of the Alzheimer’s Association is "to eliminate Alzheimer’s disease through the advancement of research; to provide and enhance care and support for all affected; and to reduce the risk of dementia through the promotion of brain health." The Alzheimer's Association, New York City Chapter provides a variety of education, information and support services to persons with dementia, their families, health care professionals, and the community.
Under the supervision of the Senior Care Consultant, the Care Consultant is part of the care and support program team. Care consultants are responsible for delivering programs and services to meet the needs of persons with Alzheimer's disease and dementia, their families and caregivers, professionals, paraprofessionals and community members in accordance with the Alzheimer’s Association Strategic Plan and Common Program Portfolio. The Care Consultant provides information, support and problem solving counsel to individuals living with Alzheimer’s or a related dementia, their families and caregivers including priority diverse populations among Latino, African American and LGBT constituencies. As such, the Care Consultant plays a critical role in reaching and serving the underserved communities across the five Boroughs. The Care Consultant works with individuals or families, in-person or over the phone, who have care or dementia related questions by providing problem-solving strategies, disease education, decision-making support, crisis assistance, guidance during transitions and connections to local services. The Care Consultant will also be responsible for the management of our Medic Alert and Safe Return program which is a 24 hour support system to assist families in the event that their loved one with dementia wanders. Duties under this program will include regular communication with the Medic Alert staff members, on-call hours until 9pm Monday to Friday, and rotating weekends hours to respond to emergency calls. He/She will also be responsible for managing support groups throughout the five boroughs including recruiting, managing, and training volunteer support group facilitators. As part of their training, the Care Consultant will conduct education activities to meet the needs of persons with Alzheimer's disease and dementia, their families and caregivers. Educational activities conducted by the Care Consultant may continue beyond the initial training period on an as needed basis.
ESSENTIAL JOB FUNCTIONS:
The function of the Care Consultant is to advance the Care and Support arm of the Association’s mission. Under the supervision of the Senior Care Consultant , the Care Consultant will deliver designated clinical/social services programs:
CARE AND SUPPORT
?Provide care consultations by providing support, information, problem solving techniques and planning for future needs. Conduct assessment of constituent needs and create action plans/recommendations based on identified needs. This may also include the provision of resource materials to constituents as needed.
?Care consultation to individuals and families, on the phone and in person, in a professional, culturally competent and compassionate manner. Empower constituents to take action as a result of receiving support, advice and counsel.
?Provide follow-up to our 24/7 Helpline callers to provide disease information, information on local resources/programs, referrals, and general follow-up as needed.
?Provide follow-up, emotional support and crisis management to families enrolled in our Medic Alert and Safe Return Program. Some evening and weekend hours will be necessary.
?Educate constituents on Alzheimer’s disease or related dementia, care giving strategies, communication skills and dementia related behaviors. Education may be one on one or group education. Link constituents to emergency, chapter and/or community resources.
?Recruit, train, support and/or coordinate a volunteer workforce to lead support groups within the designated counties. Groups may be held in person, over the phone, or virtually and will include emotional support, socialization and/or education.
?Document all contacts with constituents into our data system in a timely matter. Complete monthly reports to reflect progress/barriers to progress each month, record all Medic Alert and Safe Return cases, and submit any other reports to supervisor by set deadlines.
?Increase and coordinate development and implementation of services to diverse communities and populations in the region.
?Work collaboratively with the Programs team to secure program support via grants and assist with the program delivery, data collection and preparation and submission of reports for grantors and other stakeholders
?Conduct outreach and education with physicians, builds working relationships with service providers, care facilities and other stakeholders who collaborate on Association initiatives as necessary.
?Remains knowledgeable about current in information related to Alzheimer’s disease and related dementias as well as national, state, and local initiatives that affect the targeted population
?Work collaboratively with chapter staff to support the Alzheimer’s Association’s vision, mission, core values and strategic goals and promote Association programs such as TrialMatch® and Advocacy.
?Participate in, and assist with insuring the success of The Longest Day, Walk to End Alzheimer’s and other Alzheimer’s Association special events.
?Assist Senior Care Consultant in identify business opportunities; strengthen relationships with key individuals, organizations and businesses. Build relationships that will enhance client services and assist in growing service contacts. Report on outreach and relationship building activities.
?Represent the chapter at appropriate and assigned meetings and events.
?Collaborate with staff for referral process and other Chapter activities.
QUALITY ASSURANCE, MONITORING, REPORTING
?Administer client tracking system from referral to closure. Provide accurate and timely documentation of interactions and interventions.
?Conform to all legal and other requirements with respect to patient records and documentation, and meet Association standards for quality services.
?Collect and enter relevant and appropriate data for grant reporting, measuring progress toward strategic goals, and submitting monthly reports to the Director of Programs and Senior Care Consultant.
?Assist liaising between Chapter, National Alzheimer’s Association, and grantors and coordinate systems for collection and reporting of data and outcomes; work with team to meet performance outcomes.
?Master's degree in Social Work or Counseling or related field. Licensure required (LMSW, LMFT, LMHC).
?Demonstrated experience providing counseling and related services
?Familiarity with Alzheimer’s and related dementias, and/or aging and health. Knowledge of Alzheimer's or dementia or other disease related experience preferred.
?Experience facilitating support groups and/or providing education.
?Experience in volunteer management and support.
?Demonstrated experience working with diverse communities including Latino, African Americans, and LGBT.
?Excellent written and oral communication skills.
?Strong public speaking skills.
?Excellent customer service skills when working with internal and external clients.
?Proficient in Windows and other software programs: Word, Excel, PowerPoint, Publisher, Personify and Google Apps
?Ability to work early evening and weekend hours as needed.
?Valid driver’s license, access to reliable vehicle, and proof of current automobile insurance
?Fluency in Spanish highly preferred.
?Solid work ethic, attention to detail, and strong time management skills
?Ability to take feedback, work semi-independently as well as work as a member of a team.
?Willingness to work some evenings and weekends and travel to care consultation and outreach activities
?Commitment to serving diverse and vulnerable constituencies including immigrants, LGBT and communities of color
