The role of Alzheimer’s Association Capital of Texas Chapter Care Consultant is responsible to provide support and education to patients and family members as it relates to the diagnosis of Alzheimer’s disease and/or related dementia diagnosis. This position will be mobile, in the Travis, Williamson, Bell, Hayes and Bastrop Counties; patients and families may be referred to the Care Consultant directly upon diagnosis or medical appointments. Providing consultations and support, education and resources, the Care Consultant ensures that each individual and their care partner receives a thorough understanding of Alzheimer’s/dementia supports and services available while also ensuring that related medical concerns are considered. In addition to individual and family care and support, the Care Consultant will work with the Capital of Texas Chapter to coordinate the community and professional education provided by the Alzheimer’s Association to ensure all families, physicians and health care professionals are provided the most current Alzheimer’s and dementia related information.
The Alzheimer’s Association Care Consultant will possess assessment expertise, acute care planning, program and resource enhancement and excellent communication skills. This position must be able to work with diverse populations and challenging individuals and circumstances while maintaining a professional attitude and demeanor. Computer skills and working knowledge of Microsoft Office and electronic health care data systems is required. English/Spanish Bi-lingual is required.
ESSENTIAL JOB FUNCTIONS:
Conduct assessments and create a social support care plan that complements medical care plan for patients and family member.
Provide onsite dementia-related resources and care consultations to patients, care partners and family members.
Refer individuals calling in from the community to the Alzheimer’s Association 24-Hour Helpline (1-800-272-3900) to receive dementia-related resources and care consultations.
Plan, coordinate and implement dementia specific educational workshops and community presentations in the community for patients, caregivers, care partners and concerned family members using Alzheimer’s Association approved curricula.
Conduct or coordinate a monthly class using the Alzheimer’s Association Living with Alzheimer’s curricula which is open to all referrals and interested community members.
Working with the patient and family, assist in planning and coordination of research options, as appropriate, using Trial Match and/or local research partnerships.
Maintain all confidential reports and case notes using the Alzheimer’s Association Personify data management systems.
Maintain all data records to support the creation of quantitative and qualitative outcomes to provide to supervising staff weekly.
Assist to maintain a current referral and resource list for health agencies, services and supports that address Alzheimer’s disease and related dementia diagnosis.
Participate in any quality assurance, utilization review, and peer review programs with respect to care and support services.
Assist Alzheimer’s Association in meeting all applicable accreditation, licensing and Best Practice standards relating to care and support services. Participate as necessary in the accreditation and licensing survey processes.
Maintain a strong and collaborative partnership with all Chapter staff by participating in the Capital of Texas Chapter team activities (staff meetings, events etc.)
Complete the new hire staff training provided by the Alzheimer’s meeting the specifications and requirements of the Association.
Train and manage approved volunteers as appropriate
Work from the Alzheimer’s Association Chapter office a minimum of one day per week depending on needs and assignments
Travel by car to attend meetings or presentations
Travel by car to perform assessments at identified location in above referenced counties
Work may be stressful during periods of high volume or tight deadlines
Weekend and evening work may be required to meet the needs of patient care partners and family members when approved by supervisor

Master’s degree from an accredited university or college in social work, psychology, gerontology or related field. Licensed Social Worker preferred.
Minimum of 1 years experience working in a medical setting required
Minimum 1 years of experience working with education programs and services
Knowledge of community resources
Demonstrated strong written and verbal communication skills
Experience in a variety of community and professional presentation settings
Knowledge about Alzheimers and related dementias
Bilingual in English/Spanish
