Care Consultants provide advice and counsel to individuals who contact the Association’s Contact Center, and are referred directly to the local chapter in accordance with approved clinical protocols. As trained professionals, Care Consultants work with callers who have care or health related questions by providing advice, problem-solving counsel regarding difficult behaviors, decision-making support, crisis assistance, guidance during transitions and connections to local services.
While the Association is currently working remotely, upon a return to the office, this position will be based at the Central Ohio Chapter’s office in Columbus, OH, with travel up to 25 % within the assigned territory.
This position is grant-funded and reports to the Program Director.
Essential functions and responsibilities include, but are not limited to:
- Conduct comprehensive assessments for individuals, families, and caregivers; document all interactions and interventions in a timely and accurate manner
- Develop action plan with supportive interventions
- Provide in-depth disease education and symptom management
- Offer empathetic, active listening, validation, normalization and decision-making support
- Provide problem solving counsel regarding behaviors
- Connect others with links to emergency, chapter, and community resources
- Provide psychosocial support and guidance during times of transition or crisis
- Conduct follow up provided as per action plan
- Act as a resource for other staff
- Provide outreach as needed
- Master’s degree in social work, psychology, or counseling. Licensure (LSW) required.
- Minimum two (2) years of progressive, professional experience in a healthcare setting, preferably in geriatrics or mental health with expertise in dementia
Knowledge, Skills and Abilities
- Professional experience conducting health screenings/evaluations, diagnostic intakes and/or crisis intervention counseling
- Direct aging and/or dementia experience
- Excellent interpersonal, verbal and written communication skills
- Ability to analyze information, devise plans, and make decisions
- Ability to assess risk and provide referrals to appropriate resources for crisis intervention
- Ability to work with and support diverse populations
- Must possess valid driver’s license, access to a reliable vehicle, and proof of insurance
- Ability and willingness to work occasional evenings and weekends as needed for the job
- Ability and willingness to travel up to 25% within the assigned territory
